Telehealth equipment is used in your home to monitor your health.. It can be equipment to measure your blood pressure, blood glucose levels or your weight. This can reduce the number of visits you make to your GP or practice nurse and unplanned visits to the hospital.

You’re taught how to do the tests on yourself and the measurements are automatically transmitted to your doctor or nurse, who can then see the information without you having to leave home. They can let you know if they have any concerns. This is very good for managing your long term health conditions such as Diabetes, COPD , stroke.

The benefits of telehealth is that it helps to improves medication management, and empowers the patient to take ownership and the lead on managing their health condition.

Package 1

Medication miss management

A great concern to health professionals and family of client/service user. The implications can be very grave from double dosing to missing medication and having adverse decline in health.

Our package assess the needs and ability and identifies suitable medication plan using appropriate aids and prompts such as telephone call prompts personal to the service user, or a pill monitor or pre programmed medication alarm watch/unit to prompt the client/service user.

Package 2

Daily monitoring of Diabetes

with self check and automated reading transmitter, Once you are registered the programme agreed by your G.P will be adhered to.

Ensuring that you are assisted to check your readings daily at set time intervals and readings sent, If your readings are not received by the monitoring centre, they will contact you for a prompt self test and transmit the readings.

Your Monitoring centre will build your profile of readings and if they in any particular day are not in the prescribed levels, you will be given advice on bringing your diabetes to acceptable levels by offering advice and solutions or refer you to see your G.P.

Package 3

Blood pressure and Copd monitoring
